Payment must be in full within 15 days from date of statement. In case of REMAKE, which means the final restoration(s) is not cemented and the lab charges is applied to the statement, payment for the case still must be in full within 15 days from date of statement. A 2% charge per month (annual
rate of 24%) will be charged on past due accounts. Orders
on past due accounts will be delivered COD with past
due balance added. All cases will be billed in stages
and will be paid in full according to stage. All cases
and items sent, remain the property of Chang, Yi-Yuan
until client's account is paid in full. A minimum of
$50.00 will be charged for returned checks. All disputes
shall be governed in all respects by California
law with venue in Los Angeles County,
with the prevailing party to recover attorney's fees,
court costs and other expenses, including actual expert
witness fees, if any, in addition to any other relief
to which prevailing party may be entitled.
CHANGE/CANCELLATION POLICY
A. Cancellation Policy
The cancellation fee can be calculated as follows:
Cancel the case no less than 15 days before the scheduled case received date: no penalty;
Cancel the case within 14 to 0 days before the scheduled case received date: 50% of the full payment.
Cancel after the confirmed case received date: 80% of the full payment.
At all events, your cancellation fee will be less than or equal to your full payment.
B. Change policy
If you want to change your pre-scheduled case after receiving confirmation, the alteration fee can be calculated as follows:
- No less than 15 days before the scheduled case received date: no charges;
- Within 14 to 0 days before the scheduled case received date: 30% of the full payment.
- After the confirmed case received date: 50% of the full payment
C. Make cancellation or change successfully
You may cancel or change the pre-scheduled case online at schedule.chang-lu.com. In case that you are not able to access the Internet in time, you can do the cancellation or the alteration on the phone and send us the written requests within the next two working days.
D. Billing for Case Cancellation and Change
Invoice is sent with the cancellation case and the cancellation fee will be shown on the statement for that month.

REMAKE POLICY
Each case is evaluated
on an Individual basis. The following criteria are used
to determine the charges for a remake case:
1. Restoration accurately fits the master model, but not intraorally. We'll charge full lab fee per unit for the remake.
2. The model or impression is questionable, and our lab is requested to continue
by the doctor. We'll charge full lab fee per unit for the remake.
3. Repreparation of
crown preps or abutment teeth. We'll charge full lab fee per unit for the remake.
4. For anterior cases, we requests
a "try-in" prior to case completion and is instructed
to continue by the doctor. We'll charge full lab fee per unit for the remake.
5. For anterior cases, we did a "try-in test crown / veneer" prior to case completion, but the shade of final restoration(s) DOES NOT match. We'll charge 40% of lab fee per unit for the remake.
6. The shade changed
from the original prescription. We'll charge full lab fee per unit for the remake.
If the final restoration(s) is not cemented and the lab charges is applied to the statement, payment for the case still must be in full within 15 days from date of statement.
WARRANTY POLICY AND LIMITATIONS
What is covered
1. Repair of dental restorations.
What is not covered
1. Cash refund for prosthesis.
2. Cost incurred for removal or insertion.
3. Repairs resulting from accident, neglect, abuse, failure of supportive tooth or tissue structures, improper adjustments or improper dental hygiene.
4. Incidental or consequential damages, including inconvenience, lost wages, chairtime or pain and suffering.
5. Chang, Yi-Yuan is not liable for any fixed dental restorations (over 5 [five] units) that has not been appropriately fitted prior to process.
6. Repairs, relines, temps, implants, and other dental restorations partially fabricated or completely fabricated by another lab other than Chang, Yi-Yuan
Conditions which must be met for limited warranty to apply
1. Prosthesis must be inserted by a licensed, practicing dentist.
2. Patient must adhere to semi-annual dental maintenance (cleaning and exam) program, in the office of a licensed and practicing dentist.
3. The maintenance schedule on this certificate must be documented by the attending dentist each visit to validate this warranty.
4. Dental restorations must be returned with model work in order for the credit to be issued.
Chang, Yi-Yuan does not warrant that such devices are fit for any particular purpose and if such disclaimer is not permitted by law, the duration of any implied warranty is limited to 90 days from the date of delivery.
